Exciting Full Time, Permanent Director of Case Management Opportunity near Houston, TX
Pay: $100, 000 $105, 000/Yearly

Director of Case Management Responsibilities:
- Take charge of the daily operations and human resource management within the Case Management department.
- Supervise the interdisciplinary care plan and discharge planning processes, ensuring services are effective and appropriate, with a focus on census management, patient care outcomes, and key care metrics.
- Advocate for patients and their families, ensuring services align with their needs and expectations.
- Offer guidance and support to Case Managers and other team members, including training on caseload management, policy interpretation, regulatory compliance, and organizational procedures.
- Build and maintain strong relationships with insurance providers, self-insured employers, case management firms, and other healthcare networks.
Director of Case Management Qualifications:
- CCM or ACM certification within one year
- RN, PT, OT, SLP, LSW to be considered (must be licensed)
- Director of Case Management must have two-three years of hospital based Case Management experience
- For other qualified healthcare professionals, a minimum of a bachelor's degree is required;a graduate degree is preferred.
