Job Description
A healthcare facility west of Rochester, NH is seeking a compassionate Director of Case Management to join their team! Details are listed below.
Pay Range: $105, 000 - $120, 000 / Annually
*Pay based on experience and qualifications
Director of Case Management Opportunity:
- Full-time, direct-hire opportunity
- Monday - Friday, Day Shift
- Occasional On-Call required
- Possibility of quarterly bonuses
Director of Case Management Responsibilities:
- Director of Case Management assumes responsibility for the day-to-day operations and human resource management of the Case Management department
- The Director of Case Management has 5 Direct Reports underneath them
- Provides guidance and support to Case Managers and other staff, including training on managing caseloads and interpreting regulations, policies, operational procedures, and objectives
Director of Case Management Qualifications:
- Must be either a Licensed Registered Nurse, Licensed Social Worker, or Therapist
- Minimum of a Bachelor's Degree Required
- 3+ years of management experience
- 3+ years of hospital-based Case Management experience, including Utilization Review and Discharge Planning experience
- CCM or ACMTM certification is preferred

- A 34-mile trail suitable for walking, biking, and cross-country skiing, close to Lake Sunapee
- The city hosts the largest antique shop in New Hampshire, featuring over 155 dealers with a wide range of items
- A public park with walking trails, a pond for ice skating in winter, and a large playground
- Science museum dedicated to space exploration, featuring interactive exhibits, a planetarium, and a replica of the Mercury-Redstone rocket
