Job Description
Outstanding critical access hospital located just outside Escanaba, Michigan is seeking a full-time, permanent Home Health RN Case Manager to join their team! Great benefits and Great Team!
Pay: $30.56-$35.83
Sign-On Bonus: $5, 000

Home Health RN Case Manager:
- Provides skilled nursing care to patient at their home
- Home Health RN Case Manager must be able to evaluate a patient's condition and provide quality care based on the patient needs
- Primarily Home Health department
- M-F, No Holidays, No Weekends
- Home Health car may be available, if not, will refund based on mileage

Home Health RN Case Manager Requirements:
- Graduate from an accredited school of Nursing
- Licensed or Eligible in the state of Michigan
- Able to tolerate physical requirements to perform patient treatment
- Total driving area is about an hour radius from center point
The Upper Peninsula Location:
- Near many state park and wildlife areas
- Beaches in the town and nearby as well
- Airport very close to town
- Plenty of local diners, bar & grills, and coffee shops
