Social Worker Opportunity:
Full-time, permanent, direct-hire
Schedule: Mon Fri, day shift
Social Worker Responsibilities:
Provide support and counseling to patients and families
Assist with discharge planning and resource coordination
Help patients navigate emotional, social, and financial challenges
Maintain accurate and timely documentation of services provided
Social Worker Qualifications:
Master's degree in Social Work (MSW) required
Must hold an active Social Work license or be willing to obtain one
