An established hospital located couple hours west of San Antonio, Texas is seeking a full time, permanent Licensed Master Social Worker to join their team!
The Licensed Master Social Worker Job Opportunity:

- According to prescribed policies and procedures of the organization including all applicable state, federal and accreditation regulations and under the general supervision of the Director of Case Management or her/his designee the LMSW is responsible to provide patients and families psychosocial care regarding emotional, social and economic needs under their hospital and/or hospice individualized plan of care.
- The LMSW will provide bereavement counseling to help family members and significant others cope with grief and loss issues related to death, for a minimum period of 13 months following the death of a hospice patient. The LMSW will develop, coordinate, supervise, implement and document the provision of the Bereavement Plan of Care related to the bereavement needs of families/significant others. The LMSW works in collaboration with all members of the healthcare team and provides education on social service related issues. All social services provided are documented timely into the appropriate electronic health record. Database to ensure compliance with current standards and policies.
- Minimum of 2-4 years in varied multidisciplinary settings in the health care field with sufficient experience in working with patients and families coping with illness
- Case management, hospice or home care experience desired
- Ability to assess patient needs and formulate individualized patient care plans to meet those needs. Ability to work in a field setting and exhibit ability to make sound judgments. Must have effective communication, comprehension, documentation, interpersonal and computer skills. Must demonstrate the ability to be able to work within a multidisciplinary team setting.
- Master s Degree in Social Work (MSW) from a school of social work accredited by the Council on Social Work Education or a Baccalaureate degree phycology, sociology, social work or related area (under the direct supervision of a MSW).
- A current Texas Social Worker license in good standing.
