A facility located less than two hours west of Valdosta, GA, in Florida, is seeking a compassionate Director of Case Management to join their team! Details listed below.


Pay Range: $95, 000 - $110, 000 / Annually


Director of Case Management Opportunity:

  • Full-time, direct-hire opportunity
  • Schedule: Monday - Friday, Day Shift
  • The Director of Case Management will report to the CEO
  • Setting: Rehabilitation Facility

Director of Case Management Responsibilities:

  • Director of Case Management assumes responsibility for the day-to-day operations and human resource management of the Case Management department
  • Provides guidance and support to Case Managers and other staff, including training on managing caseloads and interpreting regulations, policies, operational procedures, and objectives
  • Oversee the interdisciplinary plan of care and the discharge planning process to ensure the effectiveness and appropriateness of services with a central focus on census management, patient care outcomes, and key care indicators
  • The Director of Case Management will have 6 Direct Reports

Director of Case Management Qualifications:

  • BSN or BSW required
  • Must be either a Licensed Registered Nurse or Licensed Social Worker
  • Must be qualified to complete an assessment within the scope of practice of his/her independent discipline
  • 3+ years of hospital-based Case Management experience, including Utilization Review and Discharge Planning experience required
  • Current CCM or ACMTM certification is preferred
