A respected community hospital in Arkansas, just outside of Branson, Missouri, is seeking a Hospice Social Worker to join their Home Health & Hospice team.

Hospice Social Worker Opportunity:
- Full-Time, Permanent, Direct-Hire, Opportunity
- Monday - Friday, with rotating weekend on-call
- Offers a comprehensive and robust benefits package which includes the standard medical, dental, and vision coverage, as well as, Tuition Reimbursement, Disability coverage, and much more
- Be a core member of the Hospice team and provide support to patient's and family's emotional, social, and economic needs relating to the impact of a terminal illness
- Plan the transition and transportation of patients from hospitals to a home or another facility, arrange services, collaborate with doctors, nurses, and insurers
- Plan and Lead interdisciplinary meetings, facilitate the referrals for community resources, and review critical access admissions and monitor patients with regularly scheduled admission visits
- Must have an Arkansas LCSW or LMSW or be eligible to obtain one
- Must have or be eligible to obtain BLS Certification
- Centrally located community within proximity to areas for activities like fishing, hiking, camping, and more.
