Pay: $42.00-$51.21/hour
Registered Nurse Case Manager Opportunity:
- Full-time, permanent position
- Monday-Friday, working 8: 00AM-5: 00PM
- Working in either Home Health or in Hospice
- Rotating on-call every holiday
- The Registered Nurse Case Manager will collaborate with a multidisciplinary team to create and implement personalized treatment and discharge plans
- the Registered Nurse Case Manager will coordinate services that align with patient needs, payer guidelines, and clinical goals to ensure seamless care transitions
Registered Nurse Case Manager Requirements
- Registered Nurse License Required
- Atleast one year of case management experience working in a clinical setting
