Social Worker, Hospice & Palliative Care Full-Time | Central Wisconsin
A hospital near La Crosse, WI is seeking a compassionate Social Worker to join their Hospice & Palliative Care team!
Social Worker, Hospice & Palliative Care Care Schedule:
- Full-Time | Direct Hire
- Monday Friday | 8: 00 am 4: 00 pm
Social Worker, Hospice & Palliative Care Responsibilities:
- Provide psychosocial assessments, counseling, and support for patients and families
- Collaborate with an interdisciplinary team to develop and update individualized care plans
- Educate patients, caregivers, and families on palliative care, comfort measures, and the dying process
- Maintain accurate documentation, uphold ethical/legal standards, and participate in team meetings and quality initiatives
Social Worker, Hospice & Palliative Care Qualifications:
- MSW from an accredited School of Social Work
- Wisconsin Certificate from the Department of Regulations & Licensing Examination Board of Family Support Coordinators
- Minimum of 3 years of clinical experience
- Completion (or agreement to complete) hospice/volunteer training
