Travel Social Worker
A well-established hospital located in Minnesota near Brookings, South Dakota, is seeking a contracted Social Worker to join its exceptional team.
Offering $50/hour
Social Worker Opportunity:
- 13-week contract
- Schedule: 4 days a week, 8 am - 4: 30 pm
- Reports to Hospital DON and Hospice Manager
- Potential for a Permanent opportunity available
- Medical, Dental, and Vision Coverage available
- Weekly Direct Deposits
Travel Social Worker Responsibilities:
- Be a core member of the Hospice team and provide support to patients and families' emotional, social, and economic needs relating to the impact of a terminal illness
- Plan the transition and transportation of patients from hospitals to a home or another facility, arrange services, and collaborate with doctors, nurses, and insurers
- Plan and lead interdisciplinary meetings, facilitate referrals for community resources, and review critical access admissions and monitor patients with regularly scheduled admission visits
Travel Social Worker Qualifications:
- Must have a Bachelor's degree in Social Work
- Must have a valid LSW license or the eligibility to obtain one
- Previous experience in Hospice is a bonus
